The Preconception Window: What the Evidence Supports Before a Pregnancy Begins
Folic acid started before conception cut neural tube defects by 72 percent in a randomized trial. Mandatory fortification spares about 1,326 United States births a year from that defect.
Preconception health has one step with randomized trial evidence behind it, which is folic acid taken before conception. In 1,817 women at high risk, folic acid produced a 72 percent protective effect against neural tube defects. A Cochrane review of five trials and 6,708 births put the pooled risk ratio at 0.31. Most other preconception advice rests on association. The Unified Model of Tone reads this window as a question of timing.

What a neural tube defect is
Neural tube defects are malformations of the brain and spine that follow incomplete closure of the embryonic neural tube. The named forms are anencephaly, spina bifida and encephalocele. Closure is an early event, which is why supplementation is studied before conception.
How the trials defined the window
Both landmark trials fixed a start point rather than an embryological date. The British trial began supplementation before pregnancy. The Hungarian trial gave one tablet daily from at least one month before conception to the second missed menstrual period.
01The strongest item
Folic acid before conception is the one step with randomized trial evidence
Preconception advice is crowded and mostly untested. Observational studies show strong links between health before pregnancy and later outcomes. Interventions to improve diet during pregnancy have had little effect, and few have been tested before conception at all (Stephenson 2018).
One exception has randomized evidence. The Medical Research Council randomized 1,817 women at high risk after a previous affected pregnancy, at 33 centers in seven countries. Groups received folic acid, seven other vitamins, both, or neither (MRC 1991).
The relative risk was 0.28, with an interval of 0.12 to 0.71. Six neural tube defects occurred in the folic acid groups and 21 in the other two, among 1,195 completed pregnancies.
The comparison inside the trial
The other seven vitamins showed no significant protective effect, at a relative risk of 0.80. A general vitamin habit did not produce the finding. Folate did.
A second trial covered first pregnancies. Czeizel and Dudás confirmed 4,753 of them. Six neural tube defects occurred in the trace-element group and none in the folic acid group (Czeizel 1992).
What these numbers do not say
A neural tube defect is not the result of something a parent did or failed to do. Six affected pregnancies occurred among women taking folic acid in the MRC trial. Folate lowers risk across a population and decides no single pregnancy.

What the research shows
The figures below come from two randomized trials, a Cochrane review, a surveillance report and a placebo trial.
03Gametes and timing
The window has a measurable length, and it runs in months
The word window suggests a short countdown. Two direct measurements set the real scale.
Follicular growth is long. The ovulatory follicle originates from a cohort of pre-antral follicles that differentiated their theca interna 85 days earlier. The next ovulatory follicle is recruited 15 to 20 days before ovulation (Gougeon 1986).
The paternal side was measured directly
Sperm production has been timed in living men. Eleven with normal sperm concentrations drank deuterated water daily for three weeks. Labeled sperm appeared after 64 days, with a range of 42 to 76 (Misell 2006).
What the trials fixed instead
Neither figure is what the evidence acts on. The trial protocols are. One began at least one month before conception and ran to the second missed menstrual period (Czeizel 1992). No quoted day of embryology appears here.
04The public record
Fortification worked once, then stopped improving
Folic acid is one of the few nutritional findings that became policy. The United States recommended 400 micrograms daily in 1992, and from 1998 required enriched cereal grains to carry 140 micrograms per 100 grams (Williams 2015).
Nineteen surveillance programs covering 1999 to 2011 put the yield at about 1,326 births a year, with an interval of 1,122 to 1,531.
The part quoted less often
After the initial decrease, neural tube defect birth prevalence has remained relatively stable. The authors point to remaining gaps among women with lower folic acid intakes. Fortification lifted the floor once and has not kept lifting it.
What folate does not reach
The Cochrane review is explicit about the boundary. The pooled trials show no preventive effect on cleft palate, cleft lip, cardiovascular defects, miscarriage or any other birth defect (De-Regil 2015). Every interval crosses 1.
05Iodine
A strong association that a randomized trial did not confirm
Iodine is the clearest case of evidence pointing two ways.
The Avon Longitudinal Study measured urinary iodine in 1,040 first-trimester samples. Children of women below 150 micrograms of iodine per gram of creatinine more often scored in the lowest quartile for verbal IQ at age 8, at an odds ratio of 1.58. Reading accuracy gave 1.69 and comprehension 1.54 (Bath 2013). The analysis adjusted for 21 factors.
Then the trial reported
A double-blind trial randomized 832 mildly iodine-deficient pregnant women to 200 micrograms of iodine daily or placebo. Among 313 children assessed at age 5 to 6, verbal IQ was 89.5 against 90.2. Performance IQ was 97.5 against 99.1 (Gowachirapant 2017).
The authors state it plainly. Daily iodine supplementation in these women had no effect on child neurodevelopment at age 5 to 6. The trial tested mild deficiency, so it does not speak to severe deficiency.
06Iron and blood sugar
One modest signal, and one clear benefit that carries a real harm
Two further items have evidence worth stating. Neither is as strong as folate.
In rural Nepal, 676 children aged 7 to 9 were followed up from a double-blind randomized trial. Supplementation ran from early pregnancy to 3 months after birth. Children whose mothers received iron and folic acid scored 51.7 on a nonverbal intelligence test against 48.2 in controls (Christian 2010).
Two other supplement arms were null. Adjusted mean differences against control were 0.73 for iron with folic acid and zinc, and 1.00 for multiple micronutrients. Iron deficiency is prevalent in that setting, and supplementation started in pregnancy.
Glycemic control before pregnancy
A meta-analysis pooled 12 cohort studies covering 3,088 women with type 1 or 2 diabetes. Pre-pregnancy care reduced congenital malformation at a risk ratio of 0.25 and perinatal mortality at 0.34 (Wahabi 2012). First-trimester HbA1c fell by an average of 1.92 percent.
No randomized trials entered that pool. The same analysis reports a harm: pre-pregnancy care raised first-trimester hypoglycemia, at a risk ratio of 1.51. Efficacy and safety are separate readings.
07Marks above the genes
Gametes mature inside a physiology, and gene readout is patterned while they do
The DNA sequence a child inherits is settled at conception. Which stretches of it get read is not. Methylation and related marks change gene expression without altering a single base, and they are laid down while cells mature.
Mice carrying a transposable element in the agouti gene showed how far diet reaches into that layer. Dams given extra folic acid, vitamin B12, choline and betaine produced offspring with an altered phenotype, through increased CpG methylation at that locus (Waterland 2003).
The authors do not read that as a license to supplement freely. They close by noting that dietary supplementation, long presumed purely beneficial, may carry unintended influences on how epigenetic gene regulation is established in humans.
The window is a physiology rather than a date
The cells themselves take months. An ovulatory follicle comes from a cohort that differentiated its theca interna 85 days earlier (Gougeon 1986), and labeled sperm appeared after 64 days in men drinking deuterated water (Misell 2006).
Both parents sit inside that window. The paternal contribution reflects the recent months of a man’s life rather than a fixed lifelong fate, which puts the same runway in front of both partners.
Those cells mature inside whatever state a parent is in. Glucose handling, thyroid output, sleep organization and autonomic balance are all running while a follicle matures, and none of them holds still. The Unified Model of Tone treats that combined state as the environment the gamete is built in.
The model also fixes the order. Hormones are downstream. The endocrine system is downstream. A nervous system under sustained load shapes an endocrine system under sustained load, which is why this window is read here as a nervous-system season and not only a nutritional one.
What care looks like in this season
Care here is unremarkable to watch. It means examination and palpation of the spine, the pelvis and the upper neck, with sustained contact graded to the person on the table rather than a twist or a thrust. Complementary approaches are used frequently by women and their partners during this period, and the reviewed modalities appear low risk of harm (Smith 2016).
The model expects a body carrying less mechanical and autonomic load to do the slow work of gamete maturation with more in reserve. That is a claim about the state a parent is in rather than a claim about fertility. Recording autonomic variability, sleep organization and glycemic recovery across a preparation season, in the same people, would show it. Supplementation and diabetes care belong with your physician.
08The model’s claim
What the Unified Model of Tone predicts about the preconception window
Everything above is established science, including two null results and one documented harm. What follows is this model’s reading, stated as ours rather than drawn from the papers cited.
Tone, as the model states it, is memory, presence, and possibility at once. The past is not filed in an archive the body consults. It is present in how the body is organized now, and that is what this window is. An egg recruited from a cohort that began differentiating 85 days earlier, and sperm built over 64 days, arrive at conception as organization carrying history.
The model reads the decisive result above the same way. A single input succeeded because it arrived while a process was open. Folate at 400 micrograms changed an outcome and seven other vitamins did not (MRC 1991).
The prediction
From that follows a claim the trials do not make. The model predicts that preconception physiology reads as one organization rather than several independent panels. Nutrient status, glycemic regulation, autonomic regulation and sleep organization should track together, and drift should show up in dynamics before it shows up in values. Glycemic regulation is a recovery reading, how glucose rises to a demand and settles back. Autonomic regulation is a variability reading. Sleep organization is the coupling of daily rhythms. Those are the model’s canonical readouts, expressed in the measures this period offers.
That is a claim about how measures covary in one body, not a claim that a parent’s state determines a child’s neurological outcome. This is a claim about how development is organized rather than about what treatment does. If nutrient status, glycemic regulation, autonomic regulation and sleep organization are shown to move together, the unification claim is confirmed.

How the preconception window expresses tone
Every topic in this library expresses all of tone. Before conception three aspects carry the signature, because the one intervention with randomized evidence works through timing and specificity.
Time course
Follicles that will ovulate began differentiating 85 days earlier. Sperm take 64 days to appear. The window is set by those schedules.
Constraint
Neural tube closure happens once, early, and cannot be revisited later. That single fixed event is why the useful dose arrives before conception.
Input quality
Folate at 400 micrograms changed outcomes. Seven other vitamins in the same trial did not. Which input arrives decides the result.

Set point: HbA1c is a resting level, not one day’s reading. Prediction: planning a pregnancy is what makes an early start possible. These are readings of one organization rather than separate systems, which is the core claim of the Unified Model of Tone.

Questions families ask about the preconception window
What is actually proven to help before pregnancy?
Folic acid, and the evidence is randomized. A double-blind trial in 1,817 women at high risk found a 72 percent protective effect against neural tube defects, at a relative risk of 0.28. A Cochrane review of five trials and 6,708 births pooled the effect at 0.31 and graded it high quality. A second trial found the same protection for first pregnancies. Nothing else in preconception care has evidence of that strength, so it leads this page rather than lifestyle advice.
When should folic acid start, and how much?

Subgroup analysis found no added benefit from doses above 400 micrograms, so starting early matters more than taking more. Does folic acid prevent other birth defects?
No, and the review is explicit about it. Across the pooled trials there was no preventive effect on cleft palate, cleft lip, congenital cardiovascular defects, miscarriage or any other birth defect. Every one of those confidence intervals crosses 1. The authors conclude that folate prevents neural tube defects without a clear effect on other birth defects. Keeping the claim narrow is what makes it hold up. Widening it into a general promise is the most common error in preconception writing.
Should I take iodine before or during pregnancy?
The evidence points two ways and both readings belong here. A UK cohort of 1,040 women found that low first-trimester iodine status predicted lower verbal IQ and reading scores at ages 8 and 9, adjusted for 21 factors. A randomized trial then gave 832 mildly iodine-deficient women 200 micrograms daily and found no effect on child neurodevelopment at age 5 to 6. That trial tested mild deficiency, so it does not answer the question for severe deficiency. Ask your physician.
What if I have diabetes and want to become pregnant?
This is where preconception care has its clearest benefit and a documented cost. Pooling 12 cohort studies in 3,088 women, pre-pregnancy care reduced congenital malformation at a risk ratio of 0.25 and perinatal mortality at 0.34, and lowered first-trimester HbA1c by an average of 1.92 percent. The same analysis found more first-trimester hypoglycemia, at a risk ratio of 1.51. No randomized trials were pooled, so selection remains a live explanation. Your diabetes team should manage this well before conception.
