Every chiropractic license in the United States already covers patients of any age. What separates a pediatric chiropractor is postgraduate certification, and two organizations award it. Their programs run from 200 to 400 hours. What those hours teach is examination, force graded by age, and the judgment to withhold.

Pediatric chiropractic is defined by a credential rather than by a signature adjustment

There is no pediatric thrust the way there is a Gonstead thrust or a Logan Basic contact. A pediatric chiropractor uses whichever method suits the child on the table. Logan Basic, sacro-occipital technique, Diversified, instrument adjusting and cranial work all appear in pediatric practice. The field’s reference text, Pediatric Chiropractic by Claudia Anrig and Gregory Plaugher, runs 38 chapters across 10 parts and reserves a single part for technique. The rest covers examination, pregnancy, neurology, orthopedics and patient safety.

“There’s a difference between a chiropractor who sees kids and a pediatric chiropractor.” Shannon Good, who teaches at Life University and advises its student chapter, puts the distinction that way.

The license is the floor and the certification is the distinction

A license permits a chiropractor to see a two-week-old infant. A certification documents the hours, the supervised practice and the examination behind that decision. Five credentials exist across the two awarding bodies, four from one and one from the other, and both organizations publish directories listing holders by name and location. The ICA Council on Chiropractic Pediatrics holds roughly 150 diplomates across the United States and Canada, a figure given by Allison Harvey, a Logan University faculty member who completed the program.

The ICPA awards the CACCP at 200 hours and the DACCP at 400

The certification runs 20 required courses, and chiropractors licensed in the United States must complete 60 of its 200 hours in person. The letters stand for Certified by the Academy Council of Chiropractic Pediatrics. The ICPA reports typical completion at 14 to 18 months and recommends finishing within three years of the first seminar. The final assessment is a written essay examination that candidates take 3 to 6 months to complete, with a separate registration fee of $500.

DACCP is the diplomate. It runs 400 hours, and the 200 hour certification is its prerequisite. Diplomate candidates also contribute cases to the ICPA’s Practice-Based Research Network and submit a paper suitable for publication in a peer-reviewed journal. Both credentials lapse without continuing postgraduate coursework and active ICPA membership.

Webster certification is the credential the public asks for by name

The ICPA’s four credentials build on one another, and most chiropractors meet the organization through Webster first. It runs 13 hours and closes with three components: an on-site practical examination, an online written examination and a principles and practice agreement. The ICPA examination is written prose rather than multiple choice

Shannon Good spent two and a half years in coursework and finished with a 40-page paper instead of a scored answer sheet. An essay tests whether a candidate can reason from principle through a case, which a multiple choice instrument cannot reach. That design choice is the ICPA’s own, and it is why candidates budget 3 to 6 months for the examination alone.

The ICA Council awards the DICCP over three years and 360 classroom hours

The International Chiropractors Association established its Council on Chiropractic Pediatrics in 1992. Its credential is the Diplomate in Clinical Chiropractic Pediatrics. The program runs 30 modules and 360 classroom hours over three years, with year one alone consisting of 8 in-person modules of 12 hours each. Coursework must be co-sponsored by a chiropractic institution accredited by the Council on Chiropractic Education. The Council also publishes the Journal of Clinical Chiropractic Pediatrics.

A separate body examines the DICCP candidate, and it examines once a year

The board certification examination is conducted by the International College of Chiropractic Pediatrics, which the ICA maintains as its testing arm. The Council’s published structure is three written and three practical sections, each before a different examiner, with a passing grade of 80 percent. Harvey’s account of her own cohort adds the shape of the three years. A year-end examination clears progression at the close of each year. A cumulative examination on all three then clears the candidate to sit for boards, which ran four days of written work followed by a day of oral examination in two-hour blocks.

The two programs weight their curricula differently

The ICA program leans heavily toward pediatric disease, differential diagnosis and pharmacology. The ICPA program leans toward technique, chiropractic principle and the perinatal period, and it owns the Webster certification. Harvey completed the ICA diplomate after seven years in practice and describes the two as answering different needs. Neither is a subset of the other, which is why a chiropractor choosing between them is choosing what to be fluent in and not choosing a level.

The ICPA was founded in 1986 because a bill would have closed chiropractic care to children under 12

The proposed Connecticut legislation carried an exemption for chiropractors holding postgraduate certification in pediatric care. It never passed. Larry Webster built the certification anyway, and every credential in this field descends from that decision. He had graduated from Logan College of Chiropractic in 1959 and served as clinic director and pediatrics instructor at Life Chiropractic College in Marietta, Georgia.

The organization grew from 500 contributing members to nearly 10,000

Webster died in 1997, and his wife Connie Webster incorporated the ICPA as a 501(c)(3) nonprofit. Jeanne Ohm became executive director in 2002, when the organization had roughly 500 contributing members and ran 10 to 20 seminars a year. Joel Alcantara joined as research director the same year. The ICPA diplomate launched in 2002 at 360 hours of its own before growing to the 400 it runs today. Membership reached 1,000 in 2003, and the first diplomate class graduated in 2006.

The magazine, the research network and the campus chapters arrived between 2004 and 2010

The quarterly Pathways to Family Wellness grew out of the member newsletter in 2004. The Practice-Based Research Network followed in 2007, and its first findings appeared in Explore: The Journal of Science and Healing in 2009. Student chapters reached chiropractic college campuses from 2010. Justin Ohm took over as executive director in 2019. The ICPA now reports membership approaching 10,000 and marks its 40th anniversary in 2026.

The pediatric examination is built on observation, because the patient cannot report

An adult examination opens with a history the patient gives. An infant examination opens with a history the parent gives and a period of watching. The practitioner sets the child down and observes before touching: which way the head rests, whether the limbs settle, whether the child tracks to one side. The practitioner carries those observations through the rest of the examination instead of resolving them on the spot.

What follows is physical and ordered. Passive range of motion at each limb, with the arms checked last because an infant’s grasp and withdrawal responses make them hardest to read. The clavicles. The hips rotated through their arc to feel for a clunk. Cervical rotation taken to each side and the two compared against each other. The cranial sutures, the frontal and temporal bones and the occiput palpated in turn. A gloved intraoral sweep reads hard palate depth, the palatine bones, the pterygoid plates and the jaw. Prone, the practitioner inspects the gluteal creases and the skin over the sacrum, where a spinal anomaly can announce itself before anything else does.

Position changes are part of the examination

A distressed infant reports nothing useful. Practitioners move the child between supine on a cushion, prone, the parent’s chest and their own lap, and they skip a segment and return to it later instead of forcing it. One recorded infant visit carries the child through roughly 85 degrees of rotation to one side while she keeps a latch on the examining finger. The latch itself holds the head engaged through the arc. Ambulatory children are read in motion. Logan University’s teaching clinic turns gait assessment into a game down the main hallway.

The curriculum’s first job is naming what does not belong in a chiropractic office

Logan University’s required pediatric course sits in trimester seven of ten and spends most of its hours on medical diagnoses, red flags and the duty to refer, from neonates through adolescence. Its pediatric clinical rotation is competitive, taking 6 students from 32 applications in one recent term. The ICA diplomate curriculum devotes whole weekends to pediatric disease and medication. The discipline treats recognition and referral as the first skill rather than the last.

Force in pediatric chiropractic is graded by age, and in an infant the contact is a sustained hold

Pediatric chiropractic runs its infant work at a magnitude adults never encounter. An infant contact is a fingertip pad held against one segment in one direction. There is no rotation, no thrust and no cavitation. The practitioner presses, holds, and waits for the child to shift her own position around the contact point. Two teaching analogies for that magnitude recur across the field: a finger against a closed eyelid, and the pressure that tests a tomato for ripeness. Candidates calibrate against a kitchen scale during training, pressing a fingertip to the pan until the reading matches.

Published numbers exist. Cooperstein and Gleberzon put the sustained contact of Logan Basic at 2 to 10 ounces. The same source sets routine newborn contacts at 15 to 45 seconds, with up to 2 minutes in an unsettled infant. A toddler needs the same force and a different visit

Magnitude barely changes between the infant and the toddler. The problem changes completely, because a toddler moves. The practitioner arranges the visit around keeping the child regulated: the parent’s lap as a table, distraction, a segment skipped and revisited, an examination broken across several minutes. By school age a manual thrust becomes available, and the practitioner still grades it

An adjustment with an audible release enters the repertoire around school age, delivered in side posture or prone with a breath cue. The grading does not stop. In one recorded visit with a 6-year-old, the practitioner tested the thoracic spine, judged the tissue too guarded to move without hurting the child, and moved to a side posture lumbar contact instead. Skeletal maturity is why the ceiling rises with age. An infant’s vertebrae are still part cartilage, and the neurocentral joints do not fuse until the early school years, so the rotational vectors that suit an adult spine are absent from infant work.

Three readings decide the force on any given contact

Size and skeletal maturity set the ceiling. Tissue response under the contact sets the depth, because the practitioner reads resistance while applying pressure. The child’s state sets the timing. Shannon Good calls the hardest skill knowing when not to adjust, and when to hold and let the body finish the work. Withholding is a decision the discipline teaches by name.

Where pediatric chiropractic meets the Unified Model of Tone

The discipline grades force by age and calls the result gentleness. The Unified Model of Tone reads gentleness as a statement about magnitude, and magnitude is only one of two variables. Under the model, tone is the integrated multiscale organization of the body’s interacting state, and every clinical input is information delivered into that organization. Force scaled to a child changes magnitude while specificity holds

The model separates two axes that are routinely collapsed. Specificity is the correspondence between the informational structure of an input and the pattern the body is holding. Magnitude is a second and independent variable, running on one continuous scale from the lightest sustained touch to the most invasive surgery. A 2 ounce contact on an infant is the magnitude that system can use, and it carries full specificity at that size.

The model’s default follows. Begin with the least invasive input that can carry the message, because a small input that fails costs the system little and tells the clinician to look elsewhere for the point of purchase. Force beyond what a system needs to receive a message degrades the message. Force short of what it needs fails to deliver it. Pediatric practice has held that as a caution for 40 years. The model holds it as a principle, and it applies to adults for the same reason it applies to children.

A developing nervous system is a system still building its map

Movement is how the body reads itself. The nervous system continuously reweights proprioceptive, vestibular and visual input by reliability, leaning on the channels that report accurately and compensating away from the ones that do not. The Unified Model of Tone extends that same weighting to autonomic output. The upper neck contributes disproportionately to the stream. The deep suboccipital muscles carry a spindle density more than ten times that of the large muscles of the trunk, and those spindles connect directly to brainstem balance and eye movement centers.

In an adult, the weighting is established and being maintained. In an infant it is being written for the first time. The model reads self-registration as the body’s continuous answer to four questions: where am I, how am I related, what whole do I belong to, and what can I become next. That fourth question covers development, learning, plasticity and repair. A child is answering all four for the first time while the tissues that generate the answers are still changing shape. On the model’s account a pediatric input is a contribution to a map under construction rather than a correction to a finished one. The window of plasticity and movement as the developmental engine carry the developmental detail.

Input quality outranks input magnitude as the variable the discipline works on

What an input does is never a property of the input alone. It is a property of the input meeting a particular tone. An infant nervous system needs very little signal to register one, so the accuracy of the information matters more than the size of the push. That is the clinical logic of adjusting a child, stated in the model’s terms.

The model locates accuracy in the reading and not in the hands. An input can only correspond to a pattern that has first been found, which makes assessment the seat of specificity. That puts the model and the credentialing bodies on the same ground. Both pathways spend the bulk of their 200 to 400 hours on examination, development and differential diagnosis, and the model says that is where the accuracy lives.

What the Unified Model of Tone predicts

The model predicts, first, that the measures a pediatric examination reads separately will covary as readings of one underlying variable. Cervical rotation symmetry, resting muscle tone, postural sway and heart rate variability should move together under a single input. If those 4 measures were shown to vary independently of one another, the unification claim is false. That is the one test with no escape route.

A second prediction concerns dose. Within a single child there is a magnitude above which additional force reduces rather than increases the registered change. Instrumented contacts can find that ceiling directly. The model puts it far lower in an infant than in an adult while the shape of the curve stays the same.

A third prediction concerns regulation itself. On the model’s account, tone held closer to its regulated range yields a system that registers its own state more accurately and spends less energy resolving the mismatch. The measurable expressions are joint position sense, head repositioning accuracy, postural sway on a force plate and heart rate variability.

The fourth prediction fixes the shape of that change. Those measures converge rather than simply improve. Children who start above their age-normed middle come down, children who start below it come up, and the spread across a group narrows. An input that pushes in one direction instead shifts a whole sample one way and carries half of them further from the middle. That contrast separates an intervention restoring regulation from one masking a signal, and it is measurable in any pediatric cohort with a baseline.

Two credentialing bodies and three chiropractic colleges carry pediatric chiropractic forward
