The Central Asian Steppe
Where medicine traveled on horseback, and the surest instrument was the hand.
Bariachi are the traditional bonesetters of Mongolia and the Mongolic peoples, and baria zasal is the craft they practice: contact and correction, applied by hand to fractures, dislocations, and head injury. Steppe life produced one repeating injury, the fall from a horse, and the steppe answered it with graded touch. The Unified Model of Tone reads that answer as an early reading of tone, the regulated state of the nervous system.

The documentary record
What the record shows
The steppe wrote almost nothing about its own medicine, so the record is skeletal, material, and external. Bone, frozen tombs, and outside observers carry the dates.
Lipid residues in Botai pottery from northern Kazakhstan carry the fats of mare's milk at around 3500 BCE. Bit wear on horse teeth at the same site indicates bridling. (Outram et al., 2009)
Riding and milking appear together at the start. Both of the steppe's signature therapies, the reduction of riding injuries and the mare's milk ferment, descend from that one event.
Five Yamnaya individuals dated between 3021 and 2501 calibrated BCE carry at least four of six skeletal traits of habitual riding. They come from a study of 217 skeletons at 39 sites, about 150 of them Yamnaya. (Trautmann et al., 2023)
Riding writes itself into the pelvis, the hip joint, and the vertebrae. The steppe's characteristic injury pattern is datable in bone thousands of years before any text describes it.
One of those riders healed a fracture of the uppermost sacral vertebra, and was buried at Strejnicu in Romania around 2879 to 2633 calibrated BCE. The authors read a forceful fall onto the backside as the likeliest cause. (Trautmann et al., 2023)
The fall from a horse is the problem steppe medicine answers. It is among the earliest identified cases, and the bone knit.
Herodotus, writing around 430 BCE, records that Scythians cleansed themselves after a burial. They set three poles leaning together, covered them with mats, and threw hemp seed onto red hot stones in a pit beneath. (Histories 4.73 to 4.75)
The earliest written account of a steppe bodily practice is a whole body treatment given to the grieving, through heat, vapor, and the breath.
Three deliberate trepanations appear in a Pazyryk craniological collection of 153 skulls from the Gorny Altai, dated 500 to 300 BCE. Copper traces at the cut edges point to a bronze knife, and the method was scraping. (Krivoshapkin et al., 2014)
Steppe practitioners opened the skull on purpose, placed one hole clear of the sagittal sinus, and left no sign of bone infection. Their patients survived.
Near infrared imaging shows a multi point tool used for the main lines on the tattooed woman from Pazyryk barrow 5, held at the State Hermitage Museum. A finer single point finished the detail, and two hands of unequal skill are visible. (Caspari et al., 2025)
Pazyryk practitioners graded an instrument against living skin at a controlled depth. Precise handling of the body predates writing on the steppe.
The Records of the Three Kingdoms, compiled in the third century CE, describes the Xianbei treating disease with moxibustion. Bloodletting among nomadic tribes appears earlier still, in the Book of Han. (Qu et al., 2022)
Heat applied at chosen sites on the body is recorded among proto Mongolic peoples more than a thousand years before Buddhism reached them.
Tibetan medical theory entered Mongolian regions from the sixteenth century through the Sibu Yidian, the Tibetan Four Tantras. Mongolian medicine took its three roots, heyi, xila, and badagan, from that corpus. (Qu et al., 2022)
The theory is borrowed and the direction of borrowing is documented. The bonesetter's craft is older than the frame it was later explained in.
One injury, repeated
The steppe built a medicine around the fall from a horse
Herding on horseback produces fractures, dislocations, concussion, and the slow wear of a body held in the saddle. Steppe medicine answers that narrow and repeating problem. A 2021 review from the Mongolian National University of Medical Sciences states it plainly: in a nomadic life, concussions and fractures are common from falling off horses or colliding with livestock.
The injuries sit in the skeleton before any text describes them. Trautmann and colleagues examined 217 individuals from 39 sites in 2023, roughly 150 of them assigned to the Early Bronze Age Yamnaya culture. Five of those people, dated 3021 to 2501 calibrated BCE, carried at least four of the six bone changes habitual riding produces. The six are stress reactions where muscle meets pelvis and femur, an ovalized hip socket, altered femoral shafts, worn vertebrae, and trauma from falls.
A medicine formed under those conditions has to travel. The steppe built no healing temples and kept no libraries. Its knowledge lived in memory and in trained hands, and much of it is still carried that way.
The peoples are not one people, and the sources name them separately. Scythians in the Pontic grasslands and the Altai, Xianbei and Xiongnu in Mongolia, later Mongols, Kazakhs, and Buryats. What they share is the horse and the injuries that come with it.
The Pazyryk burials
What the frozen tombs of the Altai preserved
The Pazyryk kurgans hold the best physical evidence of early steppe practice. Ice sealed the wooden burial chambers in the Altai Mountains, and Sergei Rudenko's excavations recovered bodies, felt, clothing, and horses buried in their riding gear. The tombs date to the fourth and third centuries BCE.
Three of the skulls in a Pazyryk collection of 153 carry deliberate trepanations. Krivoshapkin and colleagues found copper at the cut edges, which points to a bronze knife, and identified scraping as the technique. In one skull the opening sits behind a cranial suture, at a safe distance from the sagittal sinus.
The operations healed. No bone infection follows the openings, and the team read that as a high survival rate and as working anatomical knowledge. The technique resembles procedures set out in the Hippocratic corpus, written in Greek in the same period.
The tattoos are the other technical record. Caspari and colleagues imaged the tattooed woman from Pazyryk barrow 5 with near infrared photography in 2025. Her main lines were made with a multi point tool whose width matched the line, the fine detail with a single point, and two tattooists of unequal skill worked on her body.
The trepanation and the tattoo are each an instrument chosen for depth and site, placed by a hand that had learned what skin and bone would take. Neither is force applied to tissue. Calibration is the steppe's oldest documented skill.
Herodotus, c. 430 BCE
The first written account of a steppe treatment
Herodotus gave the fourth book of the Histories to the Scythians, and it remains the earliest surviving written description of steppe custom. He reports that after a burial the Scythians cleansed themselves. They set three poles leaning together, covered them with mats, made a pit beneath, and threw red hot stones into it.
Onto those stones went the seed of the hemp that grew wild and cultivated in their country. Herodotus compares the plant to flax and notes that Thracians wove garments from it. The vapor, he writes, replaced bathing entirely.
Herodotus presents the rite as care administered after loss. It happens in company, in heat, through the skin and the breath, at the moment a family is least settled. The oldest documented steppe therapy treats the state of the whole person rather than a part of the body.
The Scythians then take the seed of this hemp and, crawling in under the mats, throw it on the red-hot stones, where it smoulders and sends forth such fumes... The Scythians howl in their joy at the vapor-bath.

What the bonesetter of the steppe actually does
The bariachi is the bonesetter of Mongolia and the Mongolic peoples, and baria zasal is the craft. The name is exact. Baria means to come into contact with, or to massage. Zasal comes from zasah, to fix or to correct.
The bariachi works with the hands and very little else. Sh. Bold's history of Mongolian medicine describes bonesetters who took the fractured or dislocated limb in their hands, massaged it, gave it a turn or two, and sent the patient to rest. Bold also records cupping glasses made of horn, herbal compresses, and ice used as a local anesthetic where cutting was needed.
The craft is inherited rather than enrolled in. It passes inside families, learned at the elbow of an elder, and Bold notes that bonesetters were held to need an ability born in them. Ganbat and colleagues confirm that bariachi still receive no formal training, so techniques differ from one practitioner to the next.
Examination comes first and it is tactile. The bariachi reads swelling, heat, and the position of the bone through the skin, then guides displaced structure back toward its seat with graded pressure. Bonesetters of the same kind are documented among the Buryats, where Sodnompilova and Bashkuev describe the bariachi as a distinct group of healers.
Head injury belongs to the same craft. Mongolians routinely take concussion to a bariachi, and parents bring children under five. The 2021 review found thirteen documented baria zasal techniques for concussion in the Mongolian language literature.
In the context of a nomadic lifestyle, concussions and fractures are common due to falling from horses or colliding with livestock.

Tibetan theory arrived late, and the hands were already working
Mongolian medicine's theoretical frame is Tibetan, and the direction of transmission is documented. From the sixteenth century the Sibu Yidian, the Tibetan Four Tantras, entered Mongolian regions with Buddhism. Qu and colleagues describe rapid development of Mongolian medicine from that point.
With the corpus came the three roots: heyi, xila, and badagan in the Chinese literature, khii, shar, and badgan in Mongolian. Heyi is the moving force behind thought, speech, and organ function. Xila governs heat and digestion. Badagan is the heavy, cold, and sticky element. Balance among the three is health.
The writing followed the practice by centuries. Ixibalajur, who lived from 1704 to 1788, compiled the four part Ganlu Sibu in 1751. It sets out physiology, pathology, diagnosis, treatment, and medical ethics. Zhanbuldaorji, who lived from 1792 to 1855, compiled the Mengyao Zhengdian and cataloged 879 medicinal substances by source, habitat, part used, and preparation.
Manual treatment stayed the Mongolian contribution. Qu and colleagues record that the wounds of cavalry warfare advanced bonesetting, and name Mergenchurzi of the sixteenth or seventeenth century as a master of it. Bold reports that a Mongolian minister assembled thirty specialists and founded a hospital dedicated to the craft.
Suppression and revival
Bariachi practice survived the twentieth century
Mongolian traditional medicine was pushed out and came back. The Mongolian People's Republic was declared in 1921, ties with the Soviet Union tightened, and modern medicine arrived with them. Bold records a government decision of 13 March 1937 to close the traditional clinics and pharmacies.
Practice went quiet until the end of the 1980s. After the democratic transition of 1990 it revived openly, and a traditional medicine department opened at the Mongolian National University that same year with a six year course.
The clinical evidence is early and it is Mongolian. Ganbat and colleagues report a 1990 study of 94 concussion patients treated by baria zasal. Raised leukocyte and lymphocyte counts and lowered hemoglobin returned to normal after treatment, and cerebrospinal fluid pressure fell. A separate 2021 study found the treatment more effective than placebo.
The same review found no mechanism study in any PubMed indexed journal and no shared classification of techniques. A manual therapy carried for centuries in family hands is now examined by neurologists in Ulaanbaatar.
Kumis
The mare's milk the steppe treated as medicine
Kumis, called airag in Mongolia, is fermented mare's milk, and it is as old as the domestic horse. Lipid residues in Botai pottery from northern Kazakhstan carry mare's milk fats at around 3500 BCE. Bit wear on horse teeth at the same site shows the animals were bridled and probably ridden.
Lactic acid bacteria and yeasts turn the milk tart, faintly sparkling, and mildly alcoholic. Steppe peoples drank it as a staple of the summer camp and as a restorative. In the nineteenth century sanatoria across Russia and China prescribed the koumiss cure for tuberculosis, anemia, and digestive disorders.
A 2025 review in Foods reports more than 2,300 peptides and over 350 metabolites in koumiss, with antimicrobial, antioxidant, and immune modulating activity. An estimated 30 million people drink mare's milk products, most of them in Central Asia and northern China.
Mare's milk differs from cow's milk in composition. Its casein to whey ratio is near one to one, its lysozyme and lactoferrin run high, and its beta lactoglobulin runs low. Steppe herders selected the food by taste and long observation, and the chemistry turned out to match the reputation.
The convergence
What the steppe documented, and what it is called now
Steppe healers had no anatomy, no instruments, and almost no writing. They had one repeating injury, generations of hands on bodies, and the freedom to test what worked. Traditions with no contact with the steppe reached the same short list of observations, which is evidence that there was a real state to observe. The Unified Model of Tone names that state tone and measures it.
- The bariachi reads swelling, heat, and the seat of a bone through the skin, then returns displaced structure with pressure graded to what the hand found. Trepanation was done by scraping, and Pazyryk tattoos by a tool sized to the line.Input quality, where the character of an input decides the response.
