The First Adjustment
The founding account, the documents that survive, and the date that does not quite fit
The first chiropractic adjustment is the founding account of chiropractic. In Davenport, Iowa, Daniel David Palmer pressed on the spine of a janitor named Harvey Lillard and reported that seventeen years of deafness lifted. The profession commemorates 18 September 1895. Lillard's testimonial, printed in January 1897, points to 1896. Both documents survive. What 1895 founded was a specification: the nervous system as regulator, the spine as its structural access, and the variable the Unified Model of Tone calls tone.

The claim
The first adjustment was a test of tension, not the repair of a bone
In the Ryan Block on Brady Street in Davenport, Iowa, Daniel David Palmer pressed on the spine of a janitor named William Harvey Lillard and reported that seventeen years of deafness lifted. That is the founding account of chiropractic, and the profession commemorates it on 18 September 1895. Palmer was fifty years old and had practiced magnetic healing since 1886. Lillard maintained the building in which Palmer kept a fourth-floor office. Palmer published his own detailed telling fifteen years later, across pages 17 to 19 of The Chiropractor's Adjuster, printed by the Portland Printing House Company of Portland, Oregon in 1910 (Palmer 1910). Every popular version of the story, in every school and every lobby poster, descends from that one page.
Ask what the story commits you to. If pressing one segment of a spine altered hearing, then the spine is not a stack of blocks holding a body upright. It is a control surface wired into everything the body regulates. That is a claim about tension and signal, not about carpentry. Strip the bone talk away and what remains is testable and genuinely interesting: a mechanical input delivered at one location was followed by a change in a function measured somewhere else entirely. Whether that particular result occurred as described is a separate question, and this page treats it as one. The founding event and the founding claim are not the same thing, and only one of them has to survive.
Davenport, 1895
Two men in one building, and a healing career already nine years old
William Harvey Lillard was born on 6 September 1856 in Missouri and died on 7 September 1925 in Seattle. His death certificate, Record No. 2341 of the Washington State Board of Health, gives acute cardiac failure precipitated by chronic myocarditis as the cause, and he was buried at Lake View Cemetery. He married in 1882 and again in 1895, and he appears in the Twelfth Census of the United States in 1900. When his testimonial ran in January 1897 he gave his address as 320 West Eleventh Street, Davenport, Iowa. He was an African American working man in a Mississippi river town, and he is the only patient in the history of manual medicine whose name every practitioner knows.
Palmer was born on 7 March 1845 in Pickering, Ontario, and died on 20 October 1913. He had been a grocer and a beekeeper before he took up magnetic healing in Burlington, Iowa in 1886 and moved the practice to Davenport. He absorbed the Mesmer tradition through the Caster family clinic, collected animal bones, and taught himself anatomy to a level that shows on the page. The historian Donahue counted several hundred references to more than fifty of the best medical textbooks of the period inside the 1910 Adjuster (Donahue 1990). Palmer was self-taught and he was combative about it, but he was not uninformed. The founding account was written by a man who had read the physiology of his day and disagreed with it on purpose.

What the founder actually wrote, fifteen years after the fact
Palmer's account of the Lillard case occupies a single paragraph on page 18 of The Chiropractor's Adjuster, and it has the shape of a clinical note. He takes a history and learns that Lillard felt something give way in his back while exerting himself in a cramped, stooping position, and went deaf at that moment. He forms a hypothesis from the history. He examines and finds a vertebra racked from its normal position. He obtains consent, and he records how long that took: a half-hour of talking. Then he delivers the maneuver, using the spinous process as a lever. History, hypothesis, examination, consent, intervention, outcome. The sequence is more disciplined than the legend that grew out of it.
Notice what is careful and what is absent. The consent detail is unusual for 1910 and it reads as honest, because a man inventing a miracle does not normally pause to mention that his patient took thirty minutes to agree. But no contemporaneous clinical record survives. Palmer names no vertebra. He offers no measurement of hearing, because no bedside audiometry existed in a form he could have used. Fifteen years separate the event from the telling, and the telling appears inside a book written to establish a discipline and its author's priority over it. Read the passage for what it is: a retrospective single case report by an interested party, valuable as history and useless as proof.
Harvey Lillard, a janitor, in the Ryan Block, where I had my office, had been so deaf for 17 years that he could not hear the racket of a wagon on the street or the ticking of a watch. I made inquiry as to the cause of his deafness and was informed that when he was exerting himself in a cramped, stooping position, he felt something give way in his back and immediately became deaf. An examination showed a vertebra racked from its normal position. I reasoned that if that vertebra was replaced, the man's hearing should be restored. With this object in view, a half-hour's talk persuaded Mr. Lillard to allow me to replace it. I racked it into position by using the spinous process as a lever and soon the man could hear as before.

Lillard's own testimonial is the earliest surviving document
The oldest published account of the first adjustment was written by the patient, not by the founder. It ran under the heading DEAF SEVENTEEN YEARS in the January 1897 issue of Palmer's advertising paper The Chiropractor, issue 17, pages 1 to 4, and it carries Lillard's name and his street address (Lillard 1897). The historian Joseph C. Keating Jr. identified it as the first published telling of the case (Keating 1992). It came out of a cache of early Palmer material recovered from an elevator shaft at the Davenport campus, documents spanning roughly 1896 to 1902, the years in which the discipline was still taking shape and its founder was still changing his mind in print.
What Lillard says is more modest than what tradition says he experienced. He never uses the word cured. He says he had doctored a great deal without benefit and had given up on ear treatments. He says Palmer told him the deafness came from an injury in his spine, that the idea was new to him, and that his back had in fact been injured at the time he went deaf. He says that after two treatments he could hear quite well and that his hearing remains good. Then he adds the sentence that has caused a century of trouble for the calendar. He dates the treatment himself, and his dating does not match the date the profession later carved into its founding story.
I was deaf 17 years and I expected to always remain so, for I had doctored a great deal without any benefit. I had long ago made up my mind to not take any more ear treatments, for it did me no good. Last January Dr. Palmer told me that my deafness came from an injury in my spine. This was new to me; but it is a fact that my back was injured at the time I went deaf. Dr. Palmer treated me on the spine; in two treatments I could hear quite well. That was eight months ago. My hearing remains good.

The founding date does not survive contact with the founding document
Lillard's testimonial places the treatment in 1896, not in September 1895. He writes that Palmer told him about his spine last January, and that the treatment itself was eight months ago. The issue is dated January 1897. Counting backward from that masthead puts the first consultation in January 1896 and the treatment somewhere around the spring or early summer of that year. There is no arithmetic that reconciles eight months with 18 September 1895, which would be sixteen. Joseph Keating put the problem bluntly, observing that the profession had been left to imagine chiropractic springing into existence full-grown on a magical date (Keating 1992). The magical date is the part that fails.
Surveys of the secondary literature record proposed days of 6, 15 and 18 September, and proposed years running from 1894 through 1896, with some accounts suggesting the canonical date was set later by B.J. Palmer as the profession organized itself and needed a birthday. Ask what work the exact date is doing. If a principle about the nervous system depends on which square of a calendar it landed on, it was never a principle. September 18 is best understood as a commemorative date rather than a documented one, and saying so costs nothing at all. A discipline confident in its idea can afford an accurate founding story, and a discipline that cannot afford one should examine why.
The prehistory
Palmer was already moving vertebrae seven years before Lillard
In 1888, while practicing as a magnetic healer, Palmer reported relieving a patient by adjusting the fifth dorsal vertebra. That is seven years before the commemorated date and eight years before the date Lillard himself implies. Whatever happened in the Ryan Block, it was not the first time Palmer's hands moved a bone, and he never claimed it was. The Lillard case mattered for what he built on top of it, not because a technique arrived out of nowhere. Nothing in that record diminishes the event. It relocates it, from a lightning strike to a turning point inside a working career that was already a decade old and already convinced that specific contact beat general rubbing.
The discipline did not appear that day either. In July 1896 Palmer obtained a corporate charter for Palmer's School of Magnetic Cure. The word chiropractic came afterward, supplied by his friend the Reverend Samuel H. Weed from Greek stems meaning hand and done. The January 1897 issue of the paper shows the title changing from The Magnetic Cure to The Chiropractor, and Palmer went on acknowledging magnetic work for years after that. What the archive actually documents is growth: a healer moving, over roughly a decade, from a vitalist energy model toward a structural and neurological one, taking his vocabulary with him and revising it as he went.
T4 or C2
The evidence points to the fourth thoracic vertebra, not the upper neck
Palmer never named the segment, and the profession has argued about it for more than a century. A historical review by Wiggins and Engel in the Chiropractic Journal of Australia weighed the surviving evidence and concluded that Palmer adjusted the fourth thoracic vertebra (Wiggins and Engel 2020). Julian Valdivia Tor and Joseph Foley laid out the conflicting statements of D.D. Palmer and his son B.J. Palmer in Chiropractic History in 2015, volume 35, number 2, pages 68 to 79, under the plain title T4 versus C2. The familiar picture of an upper cervical thrust on the atlas or the axis is a later reconstruction, and the contemporary record runs against it.
Three lines of evidence converge on the mid-back. Palmer described working the fourth and twelfth dorsal segments up until 1898. Cervical adjusting was not taught at the Palmer School until 1901, on the accounts of both father and son, reportedly because Palmer was avoiding a fight with local physicians over neck work. Early graduates confirm the timeline: O.G. Smith, who graduated in 1899, reported that cervical adjustments were not taught during his training, and T.H. Storey, who graduated in 1901, described the same limitation on what students were shown. If the founder was not yet teaching neck work, the founding case was almost certainly not neck work.
The hard question
No accepted mechanism connects the mid-back to the cochlea
The nerve that carries hearing is the vestibulocochlear nerve, and its course is intracranial. It does not pass through the thoracic spine at any point. If Palmer adjusted the fourth thoracic vertebra, then the anatomy offers no direct pathway from his hands to the inner ear, and that objection has to be stated rather than managed. It has also been tested directly. Marc Demers, Zoran Gajic, Erik Gerretsen and Brian Budgell applied transcutaneous electrical stimulation to the paraspinal tissue at the fourth and fifth thoracic levels and measured hearing acuity before and after. They found no change. The study appeared in Chiropractic and Manual Therapies in 2014, volume 22, article 39 (Demers 2014).
There are no randomized controlled trials showing that spinal manipulation restores hearing. Some clinical literature reports associations between upper cervical dysfunction and auditory symptoms, and that remains a live and unsettled question rather than a demonstrated result. So this page makes no claim that adjusting a mid-back restores hearing, and this practice does not offer that. What survives scrutiny is narrower and far better supported: mechanical input to spinal tissue measurably changes autonomic and central state, and the nervous system treats segmental load as information. That is the claim worth carrying forward from 1895, and it is more than enough to build on.
The disclaimer
Palmer denied inventing spinal manipulation, in writing
The founder wrote that he was not the first person to replace subluxated vertebrae, because that art had been practiced for thousands of years. His claim was deliberately narrow. He said he was the first to replace displaced vertebrae by using the spinous and transverse processes as levers to rack them into normal position, and the first to build a science from that fact. Those sentences should end every argument about ownership that the manual professions have ever had with each other. The man whose name is on the door said plainly that the hands-on lineage was ancient and that what he added was method and theory.
Read that alongside the rest of this library and the picture settles. Bonesetters worked across Europe and Asia for centuries. Hippocratic writers built traction boards and wrote out reduction technique. Andrew Taylor Still had been teaching osteopathy in Missouri since the 1870s. The first adjustment belongs to a lineage rather than to a sect, and the founder said so before anyone else had the chance to. His own later reading of what he had found, which we take up in detail on the page about Palmer and tone, went further than the profession that inherited him has usually been willing to go.
Founded on tone
Palmer filed his own discovery under tone, not under bone
The title page of Palmer's 1910 textbook (Palmer 1910) carries three words beneath the title: Founded on Tone. Not founded on the subluxation. Not founded on the spine. Not founded on the adjustment. On page 7 he gives the definition, and it is the most useful sentence he ever wrote. Tone is the normal degree of nerve tension, and life, in his phrasing, is the expression of it. Disease, on that reading, is any variation from it, nerves too tense or too slack. Fifteen years after whatever happened in the Ryan Block, the founder himself classified the founding event as a tension problem rather than a positional one.
That reframes the whole story. A vertebra out of position was never the point. It was the accessible handle on something Palmer could not measure directly: the tension state of a living nervous system, which he believed altered the rate at which signals traveled along it. He was reaching, with the physics vocabulary available to him, for regulation. Our own model of tone extends that idea well past where he took it, out to fascia, fluid, breath, posture and behavior, and that extension is ours rather than his. What belongs to him, and what the 1910 title page proves beyond argument, is the choice of variable.
Life is the expression of tone. In that sentence is the basic principle of Chiropractic. Tone is the normal degree of nerve tension. Tone is expressed in functions by normal elasticity, activity, strength and excitability of the various organs, as observed in a state of health.

The Unified Model of Tone credits the finding and declines the ownership
The Unified Model of Tone reads 1895 as the year a specification was written down. The profession was founded on one proposition, stated in three parts. The nervous system is the master regulator of human physiology. The spine is its structural home. Interference with nervous system function produces systemic dysfunction, regardless of the organ or the symptom through which it shows up. That is a specification: a regulatory center, a structural access point, and a complete system of care built on the identification. Of everything that came before it, this is the antecedent the model reads as closest to itself. That is why it is named here.
Palmer had the central observation right and no way of knowing how far it extended. The principle he was describing was not the property of chiropractic at all. It is the shared foundation of every healing profession, expressing itself through the spine in his hands and through entirely different access points in everyone else's. He named something foundational and, understandably, mistook the part he could reach for the whole. The profession has spent more than a century being punished for the size of the idea rather than credited for finding it early. A principle that turns out to govern the whole body was never going to belong to whichever profession noticed it first.
The model is precise about the sense in which the nervous system is the master, and the 1895 wording is not quite it. Every tissue in the body registers its own condition and constrains its neighbors, so regulation is distributed across the whole organism. What the nervous system does uniquely is gather, model, prioritize and redistribute what the body is already registering, folding a distributed process into increasingly unified action. Every profession reaches that integrating system through some access point. A medication reaches it through the bloodstream, a conversation through perception, a massage through the skin. The spine is the highest-density access point. The cord is the communication channel of the central nervous system, and the tissues around it carry among the densest concentrations of nervous system input anywhere in the body. That is proximity, not rank.
The discovery is older than any modern profession, and every healing tradition reached pieces of it independently, which is why the model claims no priority and depends on none. What the chiropractic literature of Palmer's era called innate intelligence is what contemporary neuroscience describes as homeostatic, allostatic, predictive and self-organizing regulation across coupled biological systems. The vocabulary has modernized and the underlying claim has not moved. The specification of 1895 is his. What the model builds on it is the model's own.
The durable claim
One site, one input, and a function that changed somewhere else
The first adjustment is best remembered not as the day a bone went back into place, but as the first time a healer treated a nervous system as a tunable structure and then looked somewhere else entirely for the result. That is what this page adds to the story. Palmer's hypothesis was wrong in its anatomy and probably wrong about the ear. His experimental logic was sound: change tension at one location, then measure a function that has no obvious mechanical connection to that location. Nearly every serious study of somatic and autonomic coupling in the century since has run some version of that same design, usually without knowing whose design it was.
Ask what you commit to when you repeat the story. If you claim it proves adjustments cure deafness, you have taken on a debt the evidence cannot pay and never could. If you claim it demonstrates that a body is one regulated system, and that its regulation can be reached through its tissues, you have taken on a debt that a century of physiology has largely covered. Keep the second claim. Drop the first. Admit the date is uncertain and the segment was probably thoracic. The account gets smaller and truer at the same time, and unlike the legend, it survives being told to someone who checks.
What the record shows
The founding of chiropractic in seven dated records
- 1886. Palmer began practicing magnetic healing in Burlington, Iowa, nine years before the commemorated adjustment (Keating 1992). The founding event sits inside a working healing career, not at the start of one.
- 1888. Palmer reported relieving a patient by adjusting the fifth dorsal vertebra, seven years before the commemorated date (Wiggins and Engel 2020). Specific segmental contact predates the Lillard case.

- Fifteen years. That is the interval between the event and Palmer's published account of it in 1910. No contemporaneous clinical record survives, no vertebra is named, and no hearing measurement is given.
- 2014. Demers, Gajic, Gerretsen and Budgell stimulated paraspinal tissue at the fourth and fifth thoracic levels and measured no change in hearing acuity, Chiropractic and Manual Therapies, volume 22, article 39 (Demers 2014). The deafness claim does not survive. The claim that spinal input reaches central and autonomic state does.

Did the first chiropractic adjustment really happen on 18 September 1895?
The date is commemorative rather than documented. Palmer's own 1910 account gives no date at all. The earliest surviving record, Harvey Lillard's testimonial in the January 1897 issue of The Chiropractor (Lillard 1897), says the treatment took place eight months earlier and that Palmer first explained the spinal theory to him last January, which points to 1896. Secondary sources record proposed days of 6, 15 and 18 September and proposed years from 1894 to 1896. The event is well attested. The calendar square is not.
